DAVID PHIZACKLEA The family of the late David would like to thank family, friends and neighbours for the sympathy cards, flowers, acts of kindness and donations received, this helped so much at this terrible time. We would like to thank Monique and staff past and present at Moorgate Place for the help and care given to David. Also Musrat and staff at I Care for the support and care given over the years. The district nurses for attending to David, also the staff at Mill Hill day centre where he spent many happy years and for never forgetting him. Royal Blackburn Hospital for looking after David during his short stay and the Ambulance Service. To Judith Talbot for her comforting words and her help with arranging the service and Darwen Funeral Services for the excellent way everything was arranged. David will never be forgotten.
